Who needs a sports physical?
Mt. Blue Regional School District policy requires a physical examination and medical approval for all students in grades 7-12 who participate in school-sponsored athletics.
How often does my child need a sports physical?
- A physical is good for no longer than 2 years from the date of the exam or as recommended by the medical provider.
- The American Academy of Pediatrics recommends an annual physical and thus many physicians will specify an expiration date of only one year. You should check with your child’s physician if you have any questions.
- An updated medical clearance may be needed in order to return to activity following an injury or illness.
Do I need a special form?
Although we do not require a specific form, we must have the date of the physical and the name and signature of the medical provider. It must also specifically state clearance to participate in sports with or without restrictions.
